I try to avoid lorem, and instead using authentic text. But then again I'm more product designer, than theme designer.
Love http://middlemanapp.com for prototypes and Holder.js (http://imsky.github.com/holder/) for placeholder images (work offline, awesome :) ).
Almost scary that no one suggested http://mockflow.com which I find as the most potent tool at the moment. Not perfect, but have a great feature set without getting too complex like azure, photoshop etc.
Mockflow combined with paper and whiteboards is a winning combo.
Remove the numbers not needed.

Primary browser: Chrome Editor: Sublime text (with following languages: Slim (ruby), SCSS) Version control: Github.com
Deployment: FTP (Transmit ), Heroku or fjords.cc
For super fast static websites I really like middleman (Ruby based, similar to jekyll).
For webapps I think Ruby on Rails is great, though the testing part exhausting.
I use middleman as prototype/design-in-browser tool. But considering moving my blog and other smaller static sites to middleman. Great community behind middleman.
